A chronic medical condition is one that has lasted, or is likely to last, for six months or longer-such as asthma, diabetes, arthritis, stroke, cancer, or heart disease. Patients with ongoing health needs may benefit from structured care planning with their doctor.

A GP Management Plan (GPMP) helps organise care for chronic conditions. It identifies your health needs, outlines the services your doctor will provide, and lists actions you can take to manage your condition.

If you require support from multiple providers, your doctor may also recommend Team Care Arrangements (TCA). These coordinate care among your doctor and at least two other health or care professionals. You’ll be asked for consent before your details are shared with any other provider.
